Find a doctorThe Coordinated Care for Children with Medical Complexity (CCCMC) program provides health care when your child has multiple medical problems due to premature birth, a traumatic brain injury, or conditions such as cerebral palsy, DiGeorge syndrome or spina bifida. We help you meet their continuing needs for treatments like intravenous medicine or the use of medical equipment to help them breathe.
Through CCCMC, we arrange and organize care between hospitals, primary care doctors, pharmacies, therapists and other health care providers. We also work with community resources (such as transportation, therapy and pharmacies) to deliver coordinated care and increased accessibility to resources for your child.
