Request proxy access with LiveWell

When you’re managing your family’s care, convenient access to your child’s health information is important. Proxy access lets you see your family’s medical records, and it’s easy to request using your own account in our LiveWell app or website.

About child proxy access

Proxy access to a child’s medical record can only be granted to the child’s parent or legal guardian. If your child is 18 or older, they’ll need to grant you access to their own medical records.

While power of attorney lets you make certain decisions about your child health care, proxy access lets you see their medical information in your own LiveWell account.

To see your family’s medical records on a computer, you’ll need a LiveWell account to submit the child proxy access form. Here’s how to request proxy access on a computer.
